Adapt Integrated Health Care is looking for a Licensed Practical Nurse to join their team!

The Licensed Practical Nurse is specially trained and responsible for assessing patients and establishing the level of care required to facilitate patient satisfaction and well-being. This will reduce patient wait times and increase the performance of the clinic by delivering positive healthy outcomes. This role will help minimize the number of calls on the team desktops and allow for quick responses to patients rather than the patient having to wait for the team to call them back.
Primary Responsibilities:
- Thoughtfully and accurately schedule patients ensuring proper appointment placement and time needs.
- Attend appropriate meetings to facilitate and report on ongoing position updates.
- Experience with electronic medical records
- Good command of the English language, grammar and spelling
- Help assist the Population Health Coordinator with duties
- Documents in the electronic health record (EHR), triage assessment, plan, implementation and evaluation, follow-up.
- Communicates and collaborates with the patient and patient’s representatives and medical team to promote optimal and realistic goals and outcomes.
- Performing other tasks assigned by your supervisor
Qualifications
Required Qualifications:
- Current LPN Oregon license with no restrictions
- CPR certification
- EHR experience
- Previous experience working with patients in a health care setting; medical office and triage experience preferred.
