40Summary:
The Clinical Pharmacy Specialist will provide expert pharmaceutical care to patients with renal transplant or nephrological conditions, both in the ambulatory and inpatient settings. This role requires a deep understanding of complex transplant and renal pharmacotherapy and the ability to develop individualized treatment plans.
Responsibilities include but are not limited to:
Patient Care: Develop and implement comprehensive medication regimens for patients with renal transplant or nephrological diseases. Provide medication counseling and education to patients and their caregivers. Monitor patient response to therapy and adjust medication regimens as needed. Collaborate with healthcare providers to optimize patient outcomes.
Clinical Services: Participate in transplant evaluation and post-transplant care. Manage medication-related adverse events and drug interactions. Provide consultative services to other healthcare professionals regarding renal pharmacotherapy.
Research and Education: Stay current with the latest advancements in renal pharmacotherapy and clinical practice. Contribute to research projects related to renal transplantation and nephrology. Participate in educational programs for healthcare providers and patients.
Qualifications:
- PharmD from an ACPE accredited school of pharmacy is required
- Completion of a PGY1 and applicable PGY2 specialty residency or a minimum of 5 years of clinical experience required
- TX Pharmacist license or eligible
- Preceptor license required (must be obtained within 90 days)
- CPR - BLS required
- BPS certification required within 1 year of employment
- Prior experience related to renal physiology, pathophysiology, and pharmacology is strongly preferred
