Job Summary
The Safety Lead - Workplace Incidents (WI) is responsible for investigating, managing, and tracking worker injuries, incidents, and near misses, conducting root cause analyses, and making recommendations for risk reduction and mitigations to prevent reoccurrence and improve workplace safety. The overall goal is to assure compliance with OSHA requirements and help the MBTA and its employees reduce on-the-job hazards and prevent workplace injuries, illnesses, and deaths. This role is to support and advance worker safety initiatives at the MBTA. Integral to this role is working collaboratively with the OHS team and Safety Department and affected Department management, employees, and contractors.
Duties & Responsibilities
- Lead, direct, and conduct investigations of MBTA workplace injuries, incidents, and near misses. Investigations include but are not limited to requesting/collecting documentation, conducting site visits and employee interviews, identifying the sequence of events and conditions that led up to and caused the injury, analyzing data, performing root cause/causal factor analyses, and preparing and reviewing draft and final investigation reports.
- Lead, direct, and conduct risk assessments and root cause/causal factor analyses using the 5Whys and other methods. Analyses include evaluation and analysis of different factors, including but not limited to job factors, human factors, organizational factors, equipment/technology factors, and practical drift. Evaluate and recommend mitigations, risk control measures, and actions to remove or control hazards and other factors to prevent reoccurrence. Work with the affected Departments to create and prioritize action plans. Oversee/track implementation/completion of the action plans. Report on the progress of action plans.
- Respond to employee injury and accident/incident events on behalf of the MBTA Safety Department.
- Prepare and oversee/direct the preparation of injury investigation reports for review by management and submission to the Massachusetts Department of Labor Standards (DLS), Massachusetts Department of Public Utilities (DPU), Federal Transportation Administration (FTA), and other outside agencies.
- Respond to DLS, DPU, and other agency inquiries and information requests on employee injuries.
- Report required "reportable" injuries to DLS.
- Review MBTA's existing injury investigation processes and procedures and develop and provide recommendations to improve the quality, effectiveness, consistency, and defensibility of the process and resultant injury investigation report. Work with Safety Managers to update the injury investigation process and procedures and develop training for Safety Department employees on the updated process and procedures.
- Lead and participate in conducting, documenting, and communicating job hazard analyses (JHAs).
- Conduct trend analysis on employee injuries and near misses.
- Prepare and present injury data and trends analysis at daily, weekly, and monthly meetings and in daily, weekly, monthly, and annual reports.
- Work with Safety Department Managers to evaluate improved options for injury data documentation collection, analysis, presentation, and reporting.
- Conduct regular meetings with the MBTA Workers' Compensation Team on the OSHA 300 log, employee injury verification, and documentation.
- Work with various departments to create toolbox talk content regarding OSHA workplace safety.
- Work collaboratively with MBTA managers, supervisors, and employees to identify and mitigate hazards.
- Represent MBTA Safety on task forces and committees established to address safety, OSHA program implementation, employee needs, and other topics relevant to Authority goals and requirements, including support of the Authority's Safety Management System (SMS) and related policies and procedures.
- Participate in the evaluation, review, and content update of the MBTA OHS Plan and worker safety programs, training, policies, and procedures.
- Work closely with representatives from the DLS and DPU to investigate and resolve identified safety issues.
- Drive a company or personal vehicle to visit/inspect work sites to assist at emergencies and events, as needed.
- Perform night and weekend Safety Duty Response in rotation with other Safety staff.
- Respond, either directly or through others, to emergencies twenty-four (24) hours per day, seven (7) days per week.
- Provide internal and external customers with a courteous and professional experience.
- Uphold the rights and interests of the Authority while building and maintaining an effective relationship with employees.
- Perform related duties and projects as assigned.
Supervision- Safety Analyst as needed on assigned projects.

- Bachelor's degree from an accredited institution in the area of safety, training, or related field.
- Six (6) years of relevant worker injury investigation experience focusing on OSHA workplace safety.
- One (1) year of supervisory, managerial, and/or leadership experience.
- Four (4) years of experience in workplace safety program compliance and injury data and trends analysis.
- Documented relevant workplace safety and injury/incident investigation safety training courses such as those provided by OSHA, or equivalent safety organizations.
- Effective organizational, analytical, multitasking, time management, and interpersonal skills. Ability to demonstrate strong analytical, communication, and report-writing skills.
- Ability to effectively communicate with regulatory agencies, community organizations, and employees at all levels of the organization on technical and sensitive matters.
- Proficient in the use of Microsoft Word, Excel, PowerPoint, and other presentation software and Database applications.
- Available and able to work any and all shifts and/or locations as assigned or directed, twenty-four (24) hours per day, seven (7) days per week, including during severe weather conditions, emergencies, holiday, or other celebrations, or other circumstances that may potentially impact service or the safety of service.
- Ability to work with a diverse workforce.
- Ability to walk the Right of Way (ROW) with PPE and required equipment on track, ballast, and uneven surfaces based on the logistics of the safety task and/or emergency response.
- Ability to access areas using stairs, ladders, and ramps across the MBTA system and facilities to observe and evaluate conditions and conduct monitoring.
- Ability to complete, apply, and adhere to concepts in safety training including but not limited to SMS Fundamentals, ICS 100, 200, 300, 400, 700, and 800, as well as courses offered on websites hosted by MassDOT, FEMA, and TSI within three (3) years of employment.
- Ability to complete a Public Transportation Safety Certification Training Program (PTSCTP) within three (3) years of employment.
- Ability to perform all tasks safely with proper PPE.
- Ability to respond to Operations (E&M and Transportation) related incidents in the field.
- Ability to carry safety equipment to and from incidents/accidents on/off the ROW.
Substitutions Include
- A High School Diploma or GED with an additional seven (7) years of directly related experience substitutes for the bachelor's degree requirement
- An associate degree from an accredited institution an additional three (3) years of directly related experience substitutes for the bachelor's degree requirement
- A master's degree in a related subject substitute for two (2) years of general experience.
- A nationally recognized certification, or statewide/professional certification in a related field substitutes for one year of experience
- Occupational Health and Safety certification (e.g., SHS) and/or related OHS course certificates.
- Certified Safety Professional (CSP) or equivalent certification.
- Completed the Transportation Safety Institute (TSI) Transit Safety and Security Program (TSSP) in Rail and/or Bus.
- Knowledge of system safety and/or Safety Management Systems (SMS) principles and best practices.
- Knowledge of:
- American Public Transportation Association (APTA) standards
- ANSI/NFPA/IEEE/IEC codes, etc.
- Experience working in transportation, safety, security, or related field.
- Demonstrated understanding of the Federal Transit Administration (FTA) and/or DPU regulations.
