Mental Health Case Management Services assist individuals with serious and persistent mental illnesses. These services include assisting clients with obtaining and maintaining entitlements, linking to needed services, monitoring treatment compliance, coordinating care between service providers, providing supportive counseling, providing education on mental illness and medication, creating service plans, and documenting services provided.

Mental Health Case Management Services assist individuals with serious and persistent mental illnesses. These services include assisting clients with obtaining and maintaining entitlements, linking to needed services, monitoring treatment compliance, coordinating care between service providers, providing supportive counseling, providing education on mental illness and medication, creating service plans, and documenting services provided.
Clinician II (Case manager, Counselor) Typical tasks include:
- Client assessment, treatment planning, and monitoring, including community outreach, help clients maintain treatment compliance.
- Documentation in the electronic medical record of all services provided, quarterly reviews of client progress, and of active service plans in a timely manner and in a format to meet Medicaid and DBHDS standards.
- Coordination of client services with the client, families, and service providers and advocacy on behalf of clients.
- Assist with discharge planning for clients that have been hospitalized including linkage to community resources.
- Good communication skills and knowledge of a wide variety of community resources; ability to find new resources that can be accessed by clients.
