40Summary:
This position provides support and resources to families for issues that extend beyond their clinical needs. Builds individual and family capacity by increasing health knowledge and self-sufficiency. Responds to the social and emotional needs of our families, providing guidance and support while navigating them through the health care system and community to achieve the best health outcomes. Serve as both an advocate and liaison between the patient/family, health care team, and community partners. Functions in collaboration with the treatment team within the Cook Childrens Neighborhood Clinics.
Education and Experience:
High School diploma or equivalent required with 3 years experience working in health care or working with vulnerable populations
Bachelors degree in social work strongly preferred but not required
Bilingual in Spanish and English strongly preferred
Strong verbal and written communication skills.
Strong customer service, phone, and computer skills
Familiarity with basic medical terminology
Highly organized with the ability to prioritize and demonstrate problem solving skills
Ability to work autonomously with minimal supervision
Certification/Licensure:
License in social work preferred but not required
